Preparing a Certificate Providers Statement
The second part of the LPA documentation requires for the Donor to be interviewed by a suitable person as listed on part two of the application form.
This to satisfy the Office of the Public Guardian that the decisions made are fully understood and the Donor is under no pressure to act against their own wishes.
A list of close family members and friends is formed and included in part 1 The Donors Statement, these people are contacted once the LPA is registered with the Office of the Public Guardian.
This gives the opportunity for those people to discuss any issues or concerns that they may have with the Donor. However if a list of names isn't submitted the Donor is required to attach an additional "Certificate Providers Statement".
This entails having a second interview with a different person who is listed on the approved list.
