The Lasting Power of Attorney Documents are available from the Office of the Public Guardian (OPG) and it consists of 3 statements:

Statement ...	Details contained in the statement
1. Donors Statement	Donors personal details Names of Attorneys; documentation allows for up to 2 Attorneys but any number of Attorneys can be chosen Guidance maybe given by the Donor and any conditions or restrictions on decisions made by Attorneys should be made clear in writing in this section Names of those people who should be notified once the LPA is registered
2. Certificate Providers Statement	An interview with an authorised person to confirm that the Donor is fully aware of the decisions made and their implications. (List of persons authorised to hold interviews are detailed on the form) The authorised person signs to say that the LPA document was written under no duress and fully understood
3. Attorneys Statement	Personal details of the chosen Attorneys Signatures of the Attorneys stating they fully understand their duties and when to act Signatures of Witnesses who oversee the Attorney signing the document

The documents can be completed without the help of a solicitor, but it may be advisable to seek professional advice.
